§ 117.36 APPLICATION; FEE.
   (A)   Each applicant for a masseur or masseuse permit shall file an application with the Superintendent of Police and pay a filing fee which shall not be refundable. The fee shall be set, from time to time, by the Board of Aldermen and a schedule of the fees will be on file in the City Clerk's Office.
   (B)   The application for a masseur or masseuse permit shall be in writing, signed, sworn to by the applicant, and shall contain the following:
      (1)   The name and address of the applicant;
      (2)   Social Security number;
      (3)   Applicant's height, weight, color of hair and eyes, and a full set of applicant's fingerprints (fingerprints to be taken by the Police Department);
      (4)   Written evidence that the applicant is at least 18 years of age;
      (5)   Business, occupation or employment history of the applicant for three years immediately preceding the date of application;
      (6)   Whether the applicant has ever been convicted of any crime within the last ten years, except for minor traffic violations:
      (7)   Names and address of the recognized school attended, the dates attended, and a copy of the diploma or certificate of graduation awarded to the applicant, showing the applicant has successfully completed not less than 270 hours of instruction:
      (8)   Two portrait photographs of the applicant at least two inches by two inches;
      (9)   A physician's certificate issued by a physician licensed to practice in this state, showing that the applicant has been examined and found to be free of contagious or communicable disease and showing that the examination was conducted within 30 days prior to the submission of the application. The physical examination shall be in addition to any normal procedures and shall include screening for tuberculosis with tuberculin skin testing and a chest x-ray; and
      (10)   Such other information as may be necessary in order for the Superintendent of Police to make any determination required by this chapter.
