§ 97.056 APPLICATION FOR PERMIT.
   Any person, firm or corporation desiring to cut any street pavement, curb or gutter in the streets or alleys of the city or desiring to cut the sidewalks of the city, or desiring to make excavations in the unpaved streets or alleys of the city shall first make application to the City Building Commissioner for a permit to do so. The application shall be upon a printed form prescribed by the Board of Public Works and Safety of the city, and shall be in duplicate, 1 copy of the permit to be retained by the City Clerk-Treasurer as a permanent record. The application shall specify the size and character of the cut to be made, and in the case of unpaved streets and alleys, shall specify the character of excavation to be made. The application will need approval by the City Street Commissioner, Building Commissioner, CEO and the City Engineer, pursuant to a decision making process approved by the Board of Public Works.
