3-3-5: APPLICATION FOR PERMIT:
   A.   Application Deadlines: An application for a special event permit shall be submitted to the county with a nonrefundable fee of one hundred dollars ($100.00) for an initial application or fifty dollars ($50.00) for the renewal of an application for an event held in the past, or other amount set by a resolution of the county commission, no later than thirty (30) days before a major event or fourteen (14) days before for a minor event. An application may be filed no earlier than twelve (12) months before the event.
      1.   Late Application Fee: If an applicant fails to meet this deadline, then the applicant shall pay a twenty five dollar ($25.00) late fee.
      2.   Fundraising Event: For an event in which seventy percent (70%) or more of the proceeds go to an individual, family, or group that is in need of financial assistance due to a medical issue, natural disaster, tragic event, or similar circumstances, all fees will be waived.
      3.   Government Sponsored Event: The fee shall be waived for an event organized by the county or any other municipality.
   B.   Statement Upon Oath Or Affirmation: The person making the application shall include a statement made by the applicant under oath and affirmation that the information contained in the application is accurate and complete.
   C.   Information Required: The application shall contain and disclose:
      1.   Contact Information: The name, phone number, e-mail address, and mailing address of the applicant;
      2.   Property Location: The address and legal description of all property upon which the assembly is to be held, together with the name, residence and mailing address of the owners of record of all such property;
      3.   Purpose: A statement regarding the nature or purpose of the special event;
      4.   Duration: The total number of days and/or hours during which the assembly is to last;
      5.   Maximum Capacity: The maximum number of persons which the applicant shall permit to attend and/or participate at any time, not to exceed the maximum number which can reasonably assemble at the location of the special event. For overnight events, the maximum number of persons allowed to sleep within the boundaries of the location of the special event is determined in consultation with the zoning ordinances of the county if the special event is to continue overnight;
      6.   Maximum Ticket Sales: The maximum number of tickets to be sold, if any;
      7.   Potable Water: The plans for supplying potable water, including the source, amount available and location of outlets;
      8.   Sanitary Facilities: The plans for providing toilet and lavatory facilities including the source, number and location, type, and the means of disposing of waste deposited;
      9.   Solid Waste: The plans for holding, collecting and disposing of solid waste material;
      10.   Emergency Services: The plans shall include the name(s) of any organization providing emergency or medical services and any agreement between the applicant and the emergency services provider;
      11.   Lighting: The plans, if any, to illuminate the location of the assembly including the source and amount of power and the location of lamps;
      12.   Parking: The plans for parking vehicles including size and location of lots, points of highway access and interior roads, including routes between highway access and parking lots;
      13.   Camping Facilities: The plans for camping facilities, if any, including facilities available and their location;
      14.   Security: The plans for security, including the number of guards, their names, addresses, credentials and hours of availability;
      15.   Fire Protection: The plans for fire protection, including the number, type and location of all protective devices, including alarms and extinguishers, and the number of emergency fire personnel available to operate the equipment;
      16.   Concessions: The plans for food concessions and concessionaires who will be allowed to operate on the grounds, including the names and addresses of all concessionaires and their license or permit numbers;
      17.   Road Closures: The plans for any roads that will be closed for the event with details on how those roads will be closed and the plans to enforce those road closures;
      18.   Site Plan: A site plan showing a diagram of all gathering points and the location of parking, sanitary facilities, spectator areas, etc. (Ord. 2014-1037-O, 5-6-2014)
