§ 112.52 APPLICATION FOR LICENSE.
   Application for a license shall be made to the City Manager upon such blank forms as shall be prescribed therefor. On such application the applicant shall state his name, whether an individual, partnership or corporation, if a partnership the names and addresses of the partners composing the same; the addresses at which it is desired to maintain and operate the coin- operated musical instrument; the nature of the business conducted at such place; the make, name, model, serial number and other identifying information with reference to the particular coin- operated musical instrument or instruments which it is desired to operate; the name and address of the owner of such instrument or instruments; and such further information as the City Manager, in his discretion, may require.
