(A) All licenses shall be issued by the Clerk under the corporate seal and shall be signed by the Clerk. Each license shall state the name of the person licensed, for what purpose licensed, the place of business, the date of the license, the amount paid therefor, and the time it shall expire.
(B) Before any license shall issue, the person desiring the same shall present to the Village Treasurer the order required in § 110.03(B) and shall also pay to the Treasurer the amount required for such license, taking his or her receipt therefor; upon the presentation of said receipt to the Clerk, he or she shall issue the license; provided that, when by any ordinance it shall be necessary for the President to approve any bond, or for the President and Board of Trustees to pass upon the question of issuing any license, the Clerk shall not issue the same until so ordered by the President or President and Board, which license shall be in substance as follows.

Know ye, that _____________________, having made proper application, and having in all respects complied with the provisions of this Village Code, therefore the said ______________ is hereby authorized and licensed to _________ at _________ in the Village from _________ 20 _____, Nevertheless, this license is granted upon the express condition that if the said _____________ shall observe and obey all provisions of this Village Code which are or may be in force regulating or relating to said business, then this license shall be valid for that period; otherwise it may be revoked at the option of the President and Board of Trustees.
