§ 51.092 REVOCATION OF PERMIT.
   (A)   Any user who violates any of these rules and regulations or applicable state and/or federal regulations, or any of the following conditions which are hereby made part of every permit, whether stated therein or not, is subject to having his permit revoked:
      (1)   The user shall factually report the wastewater constituents and characteristics of his discharge.
      (2)   The user shall report significant changes in operation or in wastewater constituents and characteristics.
      (3)   The user shall allow reasonable access to his premises for the purpose of inspection or monitoring.
      (4)   The user shall comply with each and every term and condition of the permit.
   (B)   Before a permit is revoked, the user shall be sent written notice 15 days in advance of the date of a hearing by the city. The user shall have the opportunity to present evidence at the hearing. The city will notify the user in writing of his decision by no later than 15 days after the hearing.
