Wastewater discharge permits shall be expressly subject to all provisions of these rules and regulations. Permits shall contain any or all of the following:
(A) The average and maximum wastewater constituents and characteristics.
(B) Limits on rate and time of discharge and requirements for flow regulations and equalization.
(C) Requirements for installation of inspection and sampling facilities.
(D) Pretreatment requirements.
(E) Specifications for monitoring programs which may include sampling locations, frequency and methods of sampling, number, types, and standards for tests and reporting schedule.
(F) Requirements for submission of technical reports, notices of change, or discharge reports.
(1) Letter of notification. Within 30 days after promulgation of a new pretreatment standard, the city will send a letter to applicable industrial users informing them that a pretreatment standard has been promulgated and a copy of the standard is available from the city. The letter will state a brief summary of the standard and which standard industrial classification (SIC) numbers the standards apply to. As the city distributes requested copies of new pretreatment standards to industrial users, a statement acknowledging receipt of the standards by the industrial user will also be sent. This will be signed by an authorized representative of the industrial user and then returned to the city.
(2) Submission of baseline reports. The city will notify the affected industrial user of reporting requirements pursuant to 40 C.F.R. 403.12 (b).
(3) Compliance schedule progress reports. The city will notify the affected industrial user of reporting requirements pursuant to 40 C.F.R. 403.12 (c) (3).
(4) Notice of slug loadings. The industrial user shall notify the city immediately of any slug loading as defined by § 51.002 161 of these rules and regulations.
(G) Requirements for maintaining plant records relating to wastewater discharge as specified by the city and affording the city access thereto.
(H) Mean and maximum mass emission rates, or other appropriate limits when incompatible pollutants are proposed or present in the user's wastewater discharge.
(I) Additional requirements as determined by the city.
