1363.03 PERMIT REQUIRED; APPLICATION; FEE.
   (a)   Any person desiring to construct or alter any driveway or apron for a single-family shall pay a permit fee of twenty dollars ($20.00) for an apron and thirty-five dollars ($35.00) for a driveway.
   (b)   Any person desiring to construct or alter a driveway or apron for a three-family or Commercial Business shall pay a permit fee of thirty dollars ($30.00) for an apron and fifty dollars ($50.00) for a driveway. The cost for widening a driveway shall be twenty dollars ($20.00) For a single or two-family dwelling and forty dollars ($40.00) for a three- family or commercial business. No construction or work of any kind shall be done or permitted on such proposed driveway until and unless such permit is duly applied for and issued. Upon the receipt of such application upon the forms prescribed by the Building Commissioner, if the same is found to be in accordance with the terms of this chapter and upon the payment of the permit fee, the Commissioner shall issue such permit, subject to the conditions and terms of this chapter.
   (c)   Inspections: An inspection is required after forms, reinforcement and expansion joints are installed, and before any concrete can be poured.
   (d)   When fiber mesh is used for reinforcement in lieu of wire mesh, a copy of the concrete ticket listing the property address and reflecting the inclusion of fiber mesh must be provided to the Building Department or the concrete pour will be deemed not approved.
