Application for such license shall be made to the Building Commissioner and shall state the name of the applicant, the firm name, if any, and place of business, and shall be accompanied by the permit fee. Upon receipt of a completed application, a permit shall be issued and shall apply only to the premises specified in the license. Only one person or firm shall be issued a permit for a particular location unless a different job is being performed.
