The following information shall be provided by the person applying for the trash container permit required by Section 761.01:
(a) Name, home address, business address and telephone number of the owner of the business, tenant and property owner.
(b) Name, business address and telephone number of the supplier or owner of the trash container.
(c) The dimensions of the trash container, including the capacity of the trash container.
(d) A photograph of the trash container.
(e) A site plan, designating where the trash container is to be placed.
(f) Charge for the aforesaid permit shall be fifty dollars ($50.00).
