The Deputy Health Commissioner shall make an annual report to Council depicting the condition of the Health Department. This report shall be submitted in writing to Council prior to the end of the first quarter of the following year (March 31) and shall contain a minimum of the following information:
(a) A hierarchical arrangement of the entire department noting all positions, divisions, etc.
(b) A summary of Health Board and Housing Committee members including year appointed, address, term, expiration, etc.
(c) A summary of departmental personnel, including years of service, position, work record (attendance); and past, present and future (if known) salary structures.
(d) A summary of equipment including make, type, year and condition of same.
(e) A summary of the vital statistics for the City of Warren, Trumbull County. (Division of Vital Statistics)
(f) A summary of the nursing services provided to the residents of the City, e.g., communicable disease; contagion control; sexually transmitted disease; tuberculosis control; school inspections; parochial school services; clinic hours; etc. (Division of Nursing)
(g) A summary of the sanitation inspections of barber and beauty shops; food establishments; food service control; housing inspections; pool rooms; bowling alleys; dance halls; milk control; housing complaints; soft drink establishments; weed cutting; rabies control; demolitions; trailer parks; taxicabs and drivers; garbage disposal; rodent control; air pollution; complaint investigations; commercial auctions; junk cars; garage sales; working hours, etc. (Division of Environmental Health)
(h) A summary of fees and licenses collected comparing the past two years with the reported year. The fund that the moneys were deposited into shall also be noted.
(i) A summary of permits issued by the Department with the past two years presented as a comparison to the reported year.
(j) A summary of the last two years budget expenditures in a simple line-item fashion for all departmental aspects.
(k) A summary of conditions indicating the Department's progress, i.e., improved service, reduced costs, more efficient and effective operation, etc.
