Applications for certificates of public convenience and necessity, under the provisions of this chapter, shall be filed in writing with the Director of Public Service and Safety on such forms prescribed. Such application shall contain a statement of the name and address of the applicant, the character of the business to be transacted, the kind of equipment, the number of taxicabs to be used, the vehicle's Ohio registration number, the condition of the equipment and such other facts as may be required. Such applications which apply for a franchise shall be forwarded to Council for hearings and approval.
