WAIVER OF JURY TRIAL
I, , hereby acknowledge that I am the defendant in the above styled action; that the potential penalty upon conviction for the offense charged is a fine in excess of Two Hundred Dollars ($200.00) or imprisonment, or both, and I understand that I am thereby entitled to a jury trial in this matter.
After being fully advised of the nature of the charges, the potential penalty and my rights, I hereby voluntarily waive my rights to trial by jury reserving all other rights to which I am entitled.

The city attorney, municipal court clerk or the municipal judge may, in their discretion, waive a jury trial on behalf of the city.
