5.04.334: APPLICATION; FEE:
A.   Application To Be Filed With City Clerk: Applicants for a license hereunder shall file with the city clerk in duplicate a sworn application in writing on a form to be furnished by the city clerk.
B.   Contents Of Application: The application shall include, but not be limited to, the following information:
   1.   Full name, address, description, birth date, driver's license number, and social security number of each individual applicant;
   2.   Business address or addresses, if more than one;
   3.   Description and license number or other identification of any vehicle to be used;
   4.   A statement as to whether or not the applicant has been convicted of a felony, the nature of the offense and the punishment or penalty assessed therefor.
C.   Fingerprinting And Photographing: Each individual applicant for a license or permit shall submit to fingerprinting and photographing by the police department.
D.   Fee: There is levied a fee of fifty dollars ($50.00) for the dealer license and a fee of ten dollars ($10.00) for the employee license herein required.
E.   Separate License Required For Each Location: A separate regulatory license shall be required for each location, place or premises used for the conducting of a secondhand precious metals or gems business and each regulatory license shall designate the location or place of premises to which it applies. In addition, such business shall not be carried on or conducted in any other place than that designated in or by such regulatory license. Further, such regulatory license shall not be transferable.
