A. Application Shall Be Filed With The City Clerk: Applicants for a license hereunder shall file with the city clerk, in duplicate, a sworn application in writing on a form to be furnished by the city clerk.
B. Contents Of Application: The application shall include, but not be limited to, the following information:
1. The full name, description, birth date and social security number of each applicant;
2. The permanent home address and full local address of the applicant and telephone numbers;
3. Nature of business and kinds of goods to be sold, and if applicant is a farmer or truck gardener, whether said goods are produced by him on lands he owns, cultivates and controls;
4. If employed by another, the name and address of applicant's employer together with a brief description of credentials showing the exact relationship;
5. The length of time for which the right to do business is desired;
6. The place where the goods or property proposed to be sold or orders taken for the sale thereof, are manufactured or produced, where such goods or products are located at the time the application is filed and the proposed method of delivery;
7. A statement as to whether or not the applicant has been convicted of a felony, the nature of the offense and the punishment or penalty assessed therefor and a statement as to whether or not the applicant has been convicted of any, crime, misdemeanor or violation of any municipal ordinance, the nature of the offense and the punishment or penalty assessed therefor;
8. Description and license number or other identification of any vehicle to be used;
9. Whether the applicant, upon any sale or order, shall demand, accept or receive payment or deposit of money in advance of final delivery;
10. The last five (5) municipalities wherein the applicant has worked before coming to this city;
11. A letter, to be attached to the application, of authorization from the property owner of permission for applicant to sell from their private property with the exact location of where the applicant wishes to sell;
12. The state sales tax permit number, along with a copy of said permit, issued by the Oklahoma tax commission unless the applicant can prove to the clerk's satisfaction that he is not a vendor within the definition of the state sales tax code;
13. Other relevant information as may be required by the city clerk for the investigation of the applicant.
C. Investigation Fee: At the time of filing the application, a nonrefundable fee of ten dollars ($10.00) shall be paid to the city clerk to cover the cost of investigation of the facts stated therein.
D. Investigation: Upon receipt of an application for a license, the original shall be referred to the chief of police who shall cause an investigation and report to be made of any criminal record of the applicant. The chief of police shall complete his investigation within five (5) working days of receipt of the application from the city clerk.
1. If, as a result of the investigation, the applicant's criminal record is unsatisfactory and/or the facts stated in his application are found to be untrue the chief of police shall so advise the city clerk, in writing, and return the application to the city clerk who shall notify the applicant that his application is disapproved and that no license shall be issued.
2. If, as a result of the investigation, the applicant's record is satisfactory and the statements made in the application are true and correct the chief of police shall so advise the city clerk, in writing, and return the application to the city clerk, who shall upon payment of the required fee issue the license to the applicant.
E. License Fee: The license fee shall be paid to the city clerk in advance in the amount as follows:
