No person shall construct or relay any sidewalk and/or driveway approach without first obtaining a permit therefor from the Director of Public Service and Safety. Application for such a permit shall be made on the form prescribed and shall set forth the location of the improvement and the name and address of the owner of property abutting such improvement. A fee of one dollar ($1.00) shall be collected at the time such permit is issued.
