804.03 LICENSE APPLICATION AND FEE.
   (a)   Application. Each person desiring to display a coin or electrically operated amusement device shall make written application to the Mayor for a license therefor. The application shall state the name and address of the applicant, the name and address of the person who owns the device, the type and number of devices to be displayed, the number of devices to be displayed, the address of the premises where the device will be displayed, and other matters the Mayor considers pertinent.
   (b)   Fee. On receipt of a license application, the Mayor shall issue a license for the display of a coin or electrically operated amusement device after payment of a fee of twenty-five dollars ($25.00) by the applicant.
