Every applicant for a license to maintain, operate or conduct a massage establishment shall file an application under oath with the city upon a form provided by the Department of Public Safety and pay an annual license fee of $100 per year or any part thereof. Once accepted, the application shall be referred to the Wadsworth Police Department for investigation which shall include criminal record checks of the applicant and any persons employed or to be employed as massagists. Copies of the application shall, within five days, also be referred to the County Building Department and the Fire Department. The departments shall, within 30 days, inspect the premises proposed to be operated as a massage establishment and shall make written verification to the Director of Public Safety concerning compliance with the codes of the city and state that they administer and/or enforce.
