(A) An annual application for a taxicab driver's license shall be filed with the Safety Director on forms provided by the municipality, and such application shall be verified under oath and shall contain the following information:
(1) The name, address and date of birth of the applicant taxicab driver.
(2) The experience of the applicant taxicab driver in the transportation of passengers.
(3) The applicant taxicab driver's valid Ohio Bureau of Motor Vehicles drivers license number.
(4) The applicant taxicab driver shall be at least 18 years of age.
(5) The applicant taxicab driver shall not be under any suspension, revocation, or court ordered driving privileges in any jurisdiction.
(B) At the time the application is filed the applicant shall pay a fee of $25.00.
