§ 35.09 HEALTH INSURANCE.
   (A)   The city shall pay costs associated with comprehensive health insurance for all full-time employees and eligible spouses and dependents who elect coverage. For purposes of this section, the Mayor shall be considered to be a full-time employee of the city.
   (B)   The City Auditor shall pay all costs for family or individual coverage as they fall due from the various departmental health insurance accounts except that full-time employees who successfully complete the City Wellness Program shall pay $60 per pay period for family coverage or $30 per pay period for individual coverage as to employees paid bi-weekly and $130 for family coverage or $65 for individual coverage as to employees paid monthly. Full-time employees who do not successfully complete the City Wellness Program shall pay $74 per pay period for family coverage and $37 per pay period for individual coverage as to employees paid bi-weekly and $160 for family coverage and $80 for individual coverage as to employees paid monthly. These amounts shall be deducted from the employee's paychecks. Full-time city employees may elect between individual and family coverage.
