252.05 HOSPITALIZATION AND MEDICAL INSURANCE; OPTICAL AND DENTAL CARE.
   (a)   Hospitalization and Medical Insurance.
(1)   The Director of Finance is hereby authorized to pay the premiums on hospitalization and medical insurance on each full-time City employee, his or her spouse and the unmarried dependent children of such employee until such child or children attain the age of nineteen years. Payment of insurance premiums of elected officials shall also be included, subject to paragraph (a)(4) hereof.
(2)   For the purpose of this subsection, a full-time employee is an employee who works for the City 160 or more hours per month.
(3)   Payment for the expenditure referred to in this subsection shall be made from and charged against the proper applicable fund or funds of the City.
(4)   Elected officials shall pay a premium calculated annually by the Claims Administrator. Premiums for hospitalization and medical insurance coverage must be deposited quarterly in the insurance account with the City. Each Council representative may choose to have his or her quarterly salary deposited into the insurance account and pay the balance to the City or may pay the entire amount owed to the City for the next quarter. Pre-payment will be due on the first day of each quarter in order to have insurance coverage in effect.

   (b)   Optical Care.
(1)   All nonuniformed, full-time employees of the City and their immediate families, as shall be defined in a master policy, shall be extended eye care coverage with a twenty-five dollar ($25.00) deductible provision based upon an eye examination every twenty-four months, the optical program to be paid for by the City as a fringe benefit, provided that the employee shall pay the first twenty-five dollars ($25.00).
(2)   All full-time members of the Police Division and their immediate families, as shall be defined in a master policy, shall be extended eye care coverage with a twenty-five dollar ($25.00) deductible provision based upon an eye examination every twenty-four months, the optical program to be paid for by the City as a fringe benefit, provided that the employee shall pay the first twenty-five dollars ($25.00).

   (c)   Dental Care.
(1)   A dental care program for each employee now covered by the hospitalization fringe benefit program of the City shall be instituted with 100 percent coverage for City employees being paid for by the City or its carrier and with family dental health care being paid for by the City in an amount no greater than fifty percent of the premium or care implementation of this program which shall take place no later than June 30, 1982, provided that seventy-five percent of the City employees eligible agree to the participation so indicated.

(2)   The City shall furnish to full-time employees of the Police Division a dental health care plan as an addition to their insurance program presently in effect. The terms and conditions of such dental package shall be negotiated by the City with its insurance carrier, but shall provide, among other things, that the City shall pay the employee's total cost per year up to the maximum amount offered; that the City shall pay fifty percent of family coverage; and that a certain negotiated percentage of full-time employees of the Division and their families must participate and enroll in such program in order for it to become operative, all of which should be determined by the City and its health care insurance company.
