The Chief of Police is hereby authorized to grant permits for the temporary blocking of street to prohibit vehicular traffic under the following conditions:
(a) The application for the permit shall be made to the Chief of Police in a form to be approved by him and to contain at least the following information:
(1) The organization, if any, requesting the street barricade;
(2) If no organization, the names, addresses and phone numbers of at least three adults requesting the street blockage;
(3) The reason for the requested street closing;
(4) The names, addresses and signatures of those approving the street blockage;
(5) The streets to be affected;
(6) The date and hours of the requested street closing.
(b) The application for the street closing permit must be made on forms to be furnished by the Village.
(c) The permit to be granted by the Chief of Police must include the following requirement: some form of notice, to be determined by the Chief of Police, shall be made to the affected property owners, occupants or tenants of those residences, apartments or businesses to be affected on the street or streets that will be blocked. Notice posted at the end of the street or the portion thereof to be closed, at least twelve hours in advance of the closing, shall be considered sufficient notice.
