Each applicant for a license, as required by Section 733.01, shall make an application to the Director of Finance not later than thirty days prior to the advertising or offering for sale of any goods, wares or merchandise. The applicant shall make a full disclosure, under oath, of the following information:
(a) The true name of the owner of the goods, wares or merchandise to be offered for sale.
(b) The name of the operator of the sale, if the operator is a person other than the true owner.
(c) A full and complete inventory of the quantity, kind and character of the articles to be offered for sale, which inventory shall be prepared in the following manner:
(1) An itemized listing of all the articles and quantities thereof remaining unsold which have been in stock on the premises where the sale is to be conducted for a period of fifteen days or longer prior to the application for the license.
(2) An itemized listing of all articles and the quantities thereof received in stock on the premises within the fifteen days immediately preceding the application for the license.
(3) An itemized listing of all articles and the quantities which have been ordered or which will be ordered and placed in stock during the pendency of the sale which includes orders which are bona fide without cancellation privileges and shall not comprise goods, wares or merchandise purchased on consignment.
