An application for a license as required in Section 723.07 shall be made only by a proposed licensee, shall be made separately with respect to each mechanical amusement device, shall be addressed to and filed with the Director of Finance, at the office of the Director and shall be made on forms prescribed by him. Each application shall set forth the full name and address of the applicant; whether the applicant is an individual, firm, corporation or other organization; the make, type, model and serial number or other identifying symbol of the mechanical amusement device sought to be licensed; the place at which the mechanical amusement device is proposed to be maintained, exhibited or operated; and any other information as may be reasonably required by the Director.
