Each application for a license as required by Section 717.05 shall set forth the full name and address of the applicant; whether the applicant is an individual, firm, corporation, association or other organization; the location of the bowling alley proposed to be licensed; the number of individual alleys contained or maintained, or proposed to be contained or maintained in the bowling alley; and any additional information as may be required by the Director of Finance.
