Each application for a license as required by Section 711.07 shall set forth the full name and address of the applicant; whether the applicant is an individual, firm, corporation, association or organization; the citizenship of the applicant; the location of each billiard room proposed to be licensed; the number of billiard tables contained or maintained, or proposed to be contained or maintained in the billiard room; and any other information as may be required by the Director of Finance.
