(a) Application for a certificate of registration shall be upon the form issued by the City’s Director of Administration or his designated representative and shall be accompanied by proof of Workers’ Compensation insurance and liability insurance coverage for claims occasioned by personal injury and property damage. The contractor shall furnish subcontractors’ names and addresses and the names and addresses of all employees working for the firm.
(b) Evidence of repeated violation of this chapter shall be sufficient evidence to disqualify the applicant from receiving a certificate of registration.
