City of Twinsburg Building Commissioner shall review each application for residential hauler and commercial, industrial, mercantile and office building haulers to determine the suitability of such applicant to render the services of a hauler. The applicant shall furnish and file with the City the following:
(a) The name, photograph and personal history statement of all persons to be engaged in the collection of refuse in the Municipality;
(b) A policy of liability insurance issued for the life of the license or permit applied for or longer, by a responsible insurance company, approved as to sufficiency by the Building Commissioner and as to legality by the Director of Law, providing indemnity for or protection to the applicant against loss resulting from the operation of each vehicle to the extent of one million dollars ($1,000,000) on account of injury or death of one person in any one accident; two million dollars ($2,000,000) on account of injury or death of more than one person in any one accident; and no less than five hundred thousand dollars ($500,000) for property damage.
(c) A list of trucks to be engaged in such business within the Municipality to include make, model, tonnage, serial number, capacity, etc.
(d) Proof that such vehicle(s) have been inspected by a police department, garage or similar agency and that all vehicles and equipment are in operable and safe condition. (Ord. 41-2013. Passed 4-9-13.)
