147.27 MATERNITY/PATERNITY LEAVE.
   (a)   For the birth, adoption or foster care of a child, those employees who submit an application for leave and their intent to return to work at a future date, shall be eligible for unpaid Family Medical Leave as provided by the Family Medical Leave Act.
 
   (b)   Employees applying for Family Medical Leave shall adhere to the Family Medical Leave Policy as administered by the City. Application for such leave shall be submitted on a FMLA Request Form, and shall contain a statement of the expected date of birth, the date on which the leave of absence is to commence, and the intended date of return to service. Such written request shall be submitted to Human Resources at least thirty days in advance of the beginning date of the leave.
   (c)   For the birth, adoption or foster care of a child, employees may utilize up to twelve (12) weeks of their accrued sick leave. Such leave may be substituted with vacation time, comp time, personal days, holidays, and any other paid time that the employee has accrued.
   (d)   Upon return from approved maternity/paternity leave at the time set forth in the application for leave, the employee shall be entitled to reinstatement to the same position which was held prior to leave, or to an equivalent position, with no loss of seniority.
   (e)   An employee who is on maternity/paternity leave shall receive continued hospitalization, medical-surgical and major medical insurance benefits during the authorized leave of absence, for up to three months.
