731.03 MINIMUM STANDARDS.
   Each ambulance business which wishes to qualify for such subsidization plan shall first furnish to the City through its Mayor evidence of compliance with the following minimum standards of service:
   (a)    The name and address of the company and whether it is an individual, partnership or corporation. This information must be kept current by the ambulance operator.
   (b)    The operator shall list time and experience in emergency or invalid ambulance service field.
   (c)    The names and numbers of employees together with their training. All drivers and attendants are to be certified by the American Red Cross as having successfully completed the advanced First Aid Course. This information must be kept current by the ambulance operator.
   (d)    Ambulances shall carry minimal equipment recommended by the American College of Surgeons in its March-April 1967 Bulletin, "Minimal Equipment for Ambulances". Describe the type, make, model, year, size, capacity, etc. of ambulance vehicles and equipment together with the age thereof. This information must be kept current by the ambulance operator.
   (e)    Ambulance operators must have radio transmission and receiving apparatus.
   (f)    Ambulance operators must insure the public and all patients with public liability insurance coverage for motor vehicle or malpractice neglect and furnish the City with a copy of such policy, stating policy limits of at least one hundred thousand dollars ($100,000) injury to one person, three hundred thousand dollars ($300,000) injury to two or more persons and fifty thousand dollars ($50,000) property damage.
   (g)    The ambulance company must set forth basic rates to area hospitals and transportation rates out of the area.
   (h)    The ambulance operator must make books of account and financial records available to the City for its inspection at reasonable intervals.
   (i)    The ambulance operator must consider itself as an independent contractor, not as an agent or employee of the City. The ambulance operator must comply with all federal, State and County laws as well as all municipal ordinances regulating his business and employment of personnel.
   (j)    The ambulance operator must furnish the City with a written statement agreeing to save the City harmless from any acts of neglect of the ambulance operator.
   (k)    The operator's vehicles must be clearly marked and equipped as emergency vehicles with proper warning devices.
   (l)    The ambulance operator must have an emergency vehicle stationed at a place of business with twenty-four hour telephone communication to the public, which vehicle shall normally not be farther than ten miles radius from Twinsburg unless engaged in another call.
   (m)    The ambulance operator shall provide for an adequate plan of back-up service either independently or in cooperation with other ambulance operators.
