§ 112.08 FIREWORKS PERMIT APPLICATION.
   Application for permits shall be submitted to the Clerk in writing and shall state the name of the person, place of residence and age, the place where it is proposed to give such display of fireworks, and the experience the applicant has had in the discharge of fireworks.
