§ 95.02 PERMIT FOR DAMAGING, CUTTING, BORING OR INTERRUPTING SURFACE OF THE STREET.
   (A)   It shall be unlawful for any person, group of persons, corporation or government entity to invade or in any manner interrupt the surface or subsurface of any street, alley, highway or public grounds within the city limits, without first obtaining a written permit from the City Council.
   (B)   Applications for such permit shall be made to the Mayor or City Secretary during regular business hours at the City Hall. The permit application shall specifically set out the location, size of cut and purpose of the requested operation.
   (C)   A permit fee as set by City Council from time to time shall be required for the issuance of said permit.
   (D)   In the event a permit to invade or alter the street surface is granted, the applicant shall become responsible for the repair to the street, which repair shall be subject to directions and approval of the city. Such street repair shall be of a permanent nature so as to remain stable and not create a hazard to public use for a period of at least four years, and should additional repairs be required within said four-year period, such additional repairs or the cost thereof shall be the responsibility of the permit grantee.
