§ 93.08 REQUIRED APPEAL INFORMATION.
   (A)   Any appeals shall include the following information to be provided by appellant:
      (1)   Appellant’s name;
      (2)   Appellant’s mailing address for purposes of meeting notices;
      (3)   Appellant’s telephone number/manner of contacting appellant;
      (4)   Appellant’s relationship to the inoperable motor vehicle and real estate where it is situated;
      (5)   Facts supporting appellant’s appeal;
      (6)   Any relevant information which supports appellant’s appeal; and
      (7)   An affirmation that the contents of the appeal are true to the best of the appellant’s knowledge and belief.
   (B)   Such appeal shall be on a form substantially similar to Appendix B.
