Any person, firm, association, or corporation desiring a license shall make written application to the Village President and Board of Trustees in which application shall be stated the location or proposed location of the hospital or immediate care center; the purpose for which it is or is intended to be built, opened, conducted, or maintained; the accommodations or proposed accommodations for the patients thereof; the nature and kind of treatment given or proposed to be given therein; the name and address of the chief physician or surgeons; and the owners or proposed owners thereof.
