The registration form shall contain the following:
(a) The name and address of the registered owner;
(b) The make, type, and model of the bicycle;
(c) The serial number of the bicycle;
(d) The license number of the bicycle;
(e) The original date of sale;
(f) A place for the name and address of a new registered owner;
(g) A place for the signature of the registered owner;
(h) A place for the signature of the registered owner releasing his interest;
(i) A place for the signature of the new registered owner; and
(j) A place for the name of the licensing agency.
