There is hereby adopted a parking violation notice to be used by Tallmadge law enforcement officers and shall include the following information:
(a) Location of Tallmadge Police Department.
(b) Date, time and location of violation.
(c) Tallmadge Code Section violated.
(d) License number of vehicle.
(e) Schedule of fines.
