Each hauler shall submit to the Director of Public Service a monthly report specifying the amount of refuse and recyclable materials collected for the preceding month in Tallmadge and the site to which they were transported. The report shall include the following minimum information:
(a) Number of residential rubbish customers;
(b) Number of residential recycling customers;
(c) Number of commercial customers;
(d) Number of industrial customers;
(e) Tonnage of rubbish collected ;
(f) Tonnage of recyclable materials collected ;
(g) Materials collected by item in percent and tonnage : Newspaper, glass, plastic, aluminum and bi-metal ;
(h) Period covered by report ;
(i) Signature of preparer.
