739.06 REVOCATION OF PERMIT.
   (a)   If any person holding a valid permit under Section 739.03 falsifies any information when applying for such permit or fails to comply with any of the requirements or conditions of such permit then such permit shall be revoked by personal notice to the permit holder or by the sending of certified mail to the street address on the permit application, notice of the revocation.
   (b)   Each permit granted pursuant to this chapter shall expire one year from issue date unless a lesser time is clearly reflected on the permit. No hauler shall continue to operate without a valid, current permit.
   (c)   Any person whose permit is revoked pursuant to subsection (a) hereof, may, within five days of the receipt of such notice, file a written appeal of such revocation with the Director of Public Service and shall state the specific grounds for appeal.
      (1)   The Director of Public Service shall forthwith set a time and place for hearing and shall so notify the appellant;
      (2)   The decision of the Director of Public Service upholding or reversing such revocation shall be final.
