739.03 APPLICATION FOR PERMIT; ISSUANCE.
   Applicants for haulers' permits shall make written application at the office of the Director of Public Service, on the forms available in that office.
   (a)   The applicant shall provide all information required by the Director of Public Service which shall include but not be limited to:
      (1)   The name, street address and telephone number of the applicant, the business and the owner and the name of the specific person to contact regarding the permit or conduct of the business;
      (2)   The make, model, year and current registration license number of each motor vehicle which will be used to conduct the business in the City of Tallmadge;
      (3)   A description of the type of materials to be collected and removed;
      (4)   Type of customer to be served by applicant (i.e., commercial, industrial or residential);
      (5)   Name, address and telephone number of the recycling facility and all other dumping sites used by the applicant in conducting business from the City of Tallmadge;
      (6)   Names and addresses of all employees and agents working within the corporation limits.
   (b)   The applicant shall pay a permit fee of fifty dollars ($50.00). The permit fee shall be paid annually and will be accepted by the City after all other requirements for the permit have been submitted and approved.
   (c)   Each hauler shall identify each motor vehicle to be used in Tallmadge for business purposes pursuant to this Chapter with the following information:
   (d)   The permitted business name in letters not less than eight inches high;
      (1)   The business telephone number in characters not less than six inches high;
      (2)   The City-issued annual permit number;
      (3)   All required identification shall be in a prominent position readily apparent from the street.
   (e)   In the event a permitted vehicle is temporarily taken out of service for repairs, the applicant shall immediately provide the Director of Public Service with the make, year, serial number and current registration number of any substituted vehicle, as well as the appropriate length of time such substitution is expected. If a permitted vehicle is permanently taken out of service and replaced with another vehicle, an amended permit shall be obtained before operating said vehicle within the corporate limits of the City of Tallmadge.
