905.02 APPLICATION; DEPOSIT.
   Each permit for making such opening or curb cutting shall be confined to a single project and shall be issued by the Service Director or other proper administrative officer. Application shall be made on a form prescribed by the Director, giving the exact location of the proposed opening or cutting; the kind of paving, the area and depth to be excavated and such other facts as the Director may deem necessary. The permit shall be issued only after a cash deposit or bond sufficient to cover the cost of restoration, as determined by the Director, has been deposited with the Director, conditioned upon prompt and satisfactory refilling of excavations and restoration of all surfaces or curbs disturbed. The fee for such permit, payable prior to issuance of the permit, shall be fifty dollars ($50.00) plus an hourly rate for field inspection to be set at time of application.
