(a) An application shall be submitted to the Building Department upon which there shall appear the name, address and telephone number of both the owner of the property and the operator; the number of individuals to reside in the facility, the general nature of their needs and condition, and the assistance to be provided them; the name by which such facility will operate; the owner of the operation; the nature of the supervision, medical and otherwise, that will be provided; and such additional information as may be required to preserve the health, safety and welfare of the residents of such facility and the citizens of this community.
(b) The applicant shall permit the proper Municipal officers to inspect such premises.
(c) Such premises shall comply with the Fire Code and Building Code of the City and shall comply with the zoning requirements of the City insofar as the provisions of the Ohio Revised Code require.
