169.14 SEPARATION FROM EMPLOYMENT.
   (a)    Resignation/Retirement. An employee choosing to resign or retire from service with an appointing authority shall:
      (1)    Notify the employee's immediate supervisor at least two (2) weeks in advance of the effective date of resignation or at least two (2) months in advance of the effective date of retirement; and
      (2)    File a formal letter of resignation or retirement prior to the effective date of such resignation or retirement, which shall contain the effective date of the resignation or retirement and shall be signed and dated by the employee at the time submitted.
 
   (b)    Eligibility For Reinstatement/Rehire. Failure to comply with subsection (a)(1) and (a)(2) may result in ineligibility for reinstatement or rehire.
   (c)    Implied Resignation. Employees who absent themselves for more than three (3) consecutive working days without an authorized leave of absence shall be presumed to have resigned and may be removed from County service.
   (d)    Workers' Compensation. If an employee is unable to return to active pay status within one (1) year from the date of injury the employee will be separated from employment with the County of Summit.
   (e)    Exit interview. Upon receiving notification of a resignation, the appointing authority may conduct an exit interview with the employee prior to the employee's last day of work.
   (f)    County Property. An employee leaving employment with the County shall return all County property to the employee's immediate supervisor.
