915.05 SYSTEMS REQUIRING PROTECTION.
   (a)   An approved backflow prevention assembly shall be installed on each service line to a consumer's water system serving premises where any of the following conditions exist or are proposed:
      (1)   An auxiliary water supply (domestic or fire);
      (2)   An actual or potential hazard to the public potable water system;
      (3)   Internal cross connections;
      (4)   Intricate plumbing arrangements, which make it impractical to determine whether or not cross connections exist;
      (5)   Repeated history of cross connections;
      (6)   Any reason it is not possible to make a complete inspection; and
      (7)   Others specified by the Director of Sanitary Sewer Services or the Director of the Ohio Environmental Protection Agency.
   (b)   Any heating, ventilating and air conditioning (HVAC) equipment or process system with chemical additives shall have an approved backflow prevention assembly installed on the system. If said assembly is not installed, the Department of Sanitary Sewer Services may order the installation of an approved backflow prevention assembly on the service line to the premises.
   (c)   No connection shall be installed or maintained whereby an auxiliary water supply may enter the public potable or consumer's water system unless said auxiliary water supply and the method of connection and use of said supply shall have been approved by the Director of Sanitary Sewer Services and by the Director of the Ohio Environmental Protection Agency as required by Ohio R.C. 6109.13.
