Every applicant for a license shall make application in writing to the County Auditor or his designee, on such form as is provided by the County Auditor, which application shall set forth:
(a) Name and residence address of the applicant;
(b) Name under which the business is to be conducted;
(c) Name of every person owning or having a proprietary interest in the business;
(d) Location where the business is to be conducted;
(e) Application shall be made by the person who is to conduct the business;
(f) Signature of the applicant.
