No person, except a person who has been issued a certificate pursuant to Ohio R.C. 3743.02 or 3743.15 as amended, shall engage in the business of selling fireworks at wholesale or retail unless the person makes application to the fire chief of the township in which such business is to be conducted for, and is issued, a permit to sell fireworks. The application shall be made annually, be accompanied by a fee of twenty-five dollars ($25.00), to be paid to the applicable township clerk, and shall contain the following information:
(a) The name and address of the applicant;
(b) The location at which the applicant intends to offer fireworks for sale at wholesale;
(c) A description of all buildings in which the applicant intends to keep or sell fireworks;
(d) A signed statement that the applicant shall keep the building in which the fireworks are to be sold open to the public for a part of each week in at least eleven months of the year;
(e) The name of the owner of the premises upon which the applicant intends to sell fireworks; and
(f) The length of any lease for the premises upon which the applicant intends to sell fireworks, if the premises is not owned by the applicant.
