759.04 LICENSING PROCEDURE.
   (a)    Every person desiring a license for vending, peddling or soliciting within the City of Struthers, Ohio shall file an application with the Safety Service Director upon a form provided by the Safety Service Director.
   (b)    The fee to file said application shall be $50.00 and shall accompany the application.
   (c)    The application shall contain the following information:
      (1)   The name, date of birth, mailing address, permanent address, telephone number, driver's license number and social security number of the applicant and of all employees, servants or agents of the applicant.
      (2)   Federal and state employer's identification number and sales tax identification number, if applicable.
      (3)   A description of the product or service to be vended, peddled or solicited.
      (4)   Name and address of the person, firm, corporation or association for whom the vending, peddling or solicitation is being made.
      (5)   The proposed dates and times of the vending, peddling and/or soliciting.
      (6)   Two (2) photographs of the applicant taken within sixty (60) days immediately prior to the date of the filing of the application, which picture shall be two inches (2") by two inches (2") showing the head and shoulders of the applicant in a clear and distinguishing manner.
      (7)   Whether the applicant has ever been convicted of a felony violation or a misdemeanor or violation involving moral turpitude including the time and place of such conviction.
      (8)   Category "A" licensee's shall also need proof of liability insurance from an Ohio insurance company or a company with a "A" rating (or better) with limits of $1,000,000 with the City named as an additional insured and a thirty (30) day notice of cancellation of the policy and provide the make, model, year, color and license plate number of automobiles used by the applicant during the period of vending, peddling or solicitation within the City.
