It is further provided that the Safety-Service Director shall furnish the necessary license forms or application blanks governing the issuance of licenses for the vending machines on which forms the following information shall be submitted:
(a) Name of owner of the vending machine;
(b) Address of owner; and
(c) Number of machines in operation in the City.
