Every person desiring to operate a bowling alley in the City shall first obtain a license from the Safety-Service Director, which license shall be issued for a period of one year and for which license the following charge or charges shall be made: thirty-five dollars ($35.00) for the first alley, plus five dollars ($5.00) extra for each additional alley. It is further provided that prior to the issuance of such license, the applicant shall furnish the following information on the regular license application form:
(a) Name of the applicant or operator of such bowling alley;
(b) Address of the establishment;
(c) Hours of operation;
(d) Name of the owner of the premises;
(e) Address of the owner of the premises; and
(f) Days on which the bowling alley shall be open.
