711.03 APPLICATION.

   Prior to the issuance of such license, the applicant shall furnish the following information on regular license application forms:

   (a)    Name of applicant or operator of such pool room or billiard parlor;

   (b)    Address of establishment;

   (c)    Hours of operation;

   (d)    Name of the owner of the premises;

   (e)    Address of the owner of the premises; and

   (f)   Days on which such pool room or billiard parlor shall be open.
