Applications for the license required in § 114.20 shall be made to the Village President in writing, signed by the applicant, if an individual, or by the duly authorized agent thereof, if a firm or corporation, verified by oath or affidavit, and shall contain the following statements and information:
(A) The name, age and address of the applicant in the case of an individual, the persons entitled to a share in profits in the case of a co-partnership and the objects for which organized, the names and addresses of the officers and directors and, if a majority interest of the stock of the corporation is owned by one person or a nominee, the name and address of the nominee, in the case of a corporation;
(B) The citizenship of the applicant, the applicant’s place of birth and, if a naturalized citizen, the time and place of the applicant’s naturalization;
(C) The character of business of the applicant and, in case of a corporation, the objects for which it was formed;
(D) The length of time the applicant has been in business of that character or in the case of a corporation, the date when its charter was issued;
(E) The location and description of the premises or place of business which is to be operated under the license;
(F) A statement whether the applicant has applied for a similar or other license on premises other than those described in this application and the disposition of the application;
(G) A statement that the applicant has never been convicted of a crime and is not disqualified to receive a license by reason of any matter or thing contained in this chapter or any village ordinances or in the Illinois Compiled Statutes;
(H) A statement the applicant, in conducting his or her place of business, will not violate any of the laws of the village, state or nation;
(I) All license applications shall include, on forms and in the manner prescribed by the Village President, the fingerprints of the persons described in division (A) of this section and each performer; and
(J) All applications shall contain a processing fee of $10 for each person who is to be fingerprinted. No processing shall be commenced without the fee being paid.
